A New Path: Documentation Program

Designed to keep clients engaged, our six-month Documentation Program provides individuals with the accountability they need to succeed in their life of recovery. Available at any time during the recovery process, the program uses information gathering to assess an individual’s progress during this period. It can be used to supplement a continuing care program or on its own, with an option to extend to twelve months. The program includes the following components: Documentation Paperwork, Face-to-Face Meetings, Site Visits, Phone Calls, and Random Drug Testing.

 
Documentation Paperwork

Documentation Paperwork
Once individuals leave residential treatment settings, they need help integrating post-treatment recommendations into their everyday lives. We help each client develop a document that outlines their weekly commitments, which can range from simple activities such as taking yoga classes to formal obligations such as attending 12-step meetings.

 
Face-to-Face Meetings

Face-to-Face Meetings
To help them remain on track, we meet with clients regularly to review their documentation paperwork. Together, we go over each commitment and discuss whether or not it was honored; if the client is having problems meeting his or her obligations, we discuss the obstacles they may be facing and develop solutions to address the problem.

 
Site Visits

Site Visits
One of the best indicators of a potential relapse is the condition of the individual’s living environment. As such, we make it a point to periodically drop in on clients who are not living in sober houses to see if they are maintaining their homes (i.e. making their beds, doing their laundry, etc.) and take a photo to capture current conditions. If necessary, we discuss specific remedies at the next meeting.

 
Phone Calls

Phone Calls
In addition to visiting each individual’s home whenever possible, we take the time to call the client’s sponsor and, if applicable, his or her counselor or sober house manager to discuss progress as well as setbacks. Based on our findings, we work with each client to develop a plan of action to address any issues that have come up.

 
Random Drug Testing

Random Drug Testing

To ensure the program is working, we partner with Mid Valley Testing Solutions to provide comprehensive drug testing as well as lab verified results. Clients may choose one of three testing options.

1. Min 33 tests over 6 months
2. Min 26 tests over 6 months
3. Min 17 tests over 6 months
